About The Position
Always Best Care Home Health helps seniors and kupuna remain independent while receiving care in the comfort of their own homes. Our clinicians work closely with patients and families to improve mobility, reduce fall risks, and support overall well-being. We are looking for a Physical Therapist who is passionate about patient-centered care and enjoys working one-on-one with patients in a home setting.
Requirements
- Hawaii Physical Therapist license
- Minimum one year of clinical experience
- Reliable transportation
- Strong communication and organizational skills
Nice To Haves
- Home health experience preferred
Responsibilities
- Conduct physical therapy evaluations and reassessments
- Create individualized treatment plans
- Address mobility, balance, strength, and safety concerns
- Provide patient and caregiver education
- Monitor progress and adjust treatment plans as appropriate
- Coordinate care with the interdisciplinary team
- Complete Medicare-compliant documentation
