About The Position
Concierge Home Care is hiring a Home Health Physical Therapist Assistant (PTA) in Lake County, FL. This role offers flexibility, independence, and the ability to increase earnings while providing meaningful one-on-one care in patients' homes. The coverage area is Clermont / Lake County, FL, with territories designed to minimize drive time through clustered scheduling. The position utilizes a pay-per-visit model with an average full-time earning potential of $85,000+ annually, plus quarterly bonus opportunities, mileage reimbursement, and a data plan stipend. Consistent patient volume is provided.
Requirements
- Active, unencumbered Florida Physical Therapist Assistant (PTA) license
- Valid driver’s license, reliable transportation, and auto insurance
- Employment is contingent upon successful completion of an AHCA Level 2 background screening.

Responsibilities
- Provide hands-on therapeutic care following the plan of care established by the Physical Therapist
- Assist patients in improving mobility, strength, and independence
- Collaborate with patients, caregivers, and interdisciplinary teams
- Monitor patient progress and communicate updates to the supervising PT
- Complete documentation using streamlined, AI-assisted tools
