Home Health Physical Therapist Assistant
About The Position
Remedy Home Health Care is hiring a Physical Therapist Assistant (PTA) for per-diem home health work in the East LA territory, including the Bellflower, Downey, and Monterey Park areas. The PTA provides personalized, one-on-one therapy in patients’ homes to support rehabilitation, fall prevention, functional mobility, independence, and quality of life. Services are delivered under the supervision of a licensed Physical Therapist and in coordination with physicians, RNs, and the interdisciplinary care team. The position offers a flexible schedule and autonomy in managing patient visits. Full-time benefits, including health insurance and PTO, are available to eligible employees.
Requirements
- Active California Physical Therapist Assistant (PTA) license.
- Current CPR certification; AHA certification is required.
- Minimum of 1 year of clinical experience.
- Strong interest in home health, rehabilitation, and senior care.
- Excellent communication and organizational skills.
- Valid driver’s license, auto insurance, and reliable transportation.
Nice To Haves
- Home health experience preferred.
Responsibilities
- Provide in-home physical therapy services according to the established Plan of Care.
- Assist patients with therapeutic exercises, mobility training, and rehabilitation programs.
- Support senior care patients in improving strength, balance, and independence.
- Monitor, document, and report patient progress to the supervising Physical Therapist.
- Educate patients and caregivers about home exercise programs, safety, and fall prevention.
- Collaborate with physicians, RNs, and the interdisciplinary care team.
- Complete documentation using WellSky Kinnser EMR.
