This full-time position is for a Physical Therapist Assistant in Home Health, serving West or South Charlotte. The role involves managing a daily caseload of patients, ensuring timely care and documentation according to the plan of care and physician orders. Responsibilities include collecting health data, implementing care programs, documenting and executing plans of care, applying clinical reasoning for assessments, providing patient and family education, observing and reporting patient condition changes, and communicating with the multidisciplinary team. The role also requires incorporating short- and long-term goals into therapy, collaborating with physicians and PT leadership, ensuring documentation meets policy and standards, and meeting productivity targets. The assistant must perform treatments in accordance with the State PT Practice Act, Home Health Conditions, JCAHO, Medicare, OASIS, and agency policy. Additionally, the role promotes shared governance, decision-making, and research to improve outcomes, maintains current PT knowledge, integrates evidence into practice, contributes to quality PT practice, communicates effectively, collaborates with patients and families, evaluates own practice, and uses appropriate resources for safe, effective, and cost-conscious PT services.
