About The Position
All About Home Care is a Houston-based home health care agency serving Houston and surrounding communities since 2004. The agency is hiring Physical Therapist Assistants to care for patients across the Greater Houston area, including North, South, East, West, and Central Houston. PRN positions are also available in The Woodlands and Conroe. The role provides physician-prescribed physical therapy under a plan of care established by a physical therapist and includes patient evaluation, treatment, education, care planning, and documentation. Kinnser software is used for documentation.
Requirements
- Graduate from a two-year college-level program approved by the American Physical Therapy Association.
- Currently licensed in the state of Texas.

- Two years of home health experience preferred.
Responsibilities
- Understand and adhere to established policies and procedures.
- Provide physician-prescribed physical therapy under a plan of care established by a physical therapist.
- Improve or minimize patients’ residual physical disabilities.
- Help patients return to an optimum and productive level within their capabilities.
- Perform skilled procedures as ordered by a physician and according to the physical therapist-established plan of care.
- Participate with other health care personnel in patient care planning.
- Take initial histories and perform initial evaluations.
- Consult with the physical therapist regarding changes in treatment.
- Instruct patients and family members or significant others in home programs and activities of daily living.
- Participate in mandatory in-service programs and present in-service programs as assigned.
- Participate in performance improvement activities as assigned.
- Attend patient care conferences as scheduled.
- Prepare medical records and progress notes, and update care plans for each patient visit in a timely manner according to agency requirements.
