About The Position

HealthPro Heritage is hiring a full-time Home Health Physical Therapist Assistant to serve adults in Mystic, Connecticut, and surrounding areas. The role offers a home health setting and the opportunity to build your own schedule. It supports HealthPro Heritage’s partnership with Masonicare home health. The assistant helps implement treatment plans designed by Physical Therapists and provides direct patient care. The role also includes monitoring and documenting progress, educating patients and families, and coordinating care with Physical Therapists and other healthcare professionals. The estimated total annual pay range is $70,200–$86,528, with a point base rate of $45–$52 per point and a $5,000 sign-on bonus. Requirements

Associate’s degree in Physical Therapist Assisting from an accredited program.
Valid state licensure as a Physical Therapist Assistant, or license eligibility.
Strong assessment, diagnostic, and therapeutic abilities.
Excellent communication and interpersonal skills.
Proficiency with diagnostic tools, therapy equipment, relevant software, and electronic health record systems.
Ability to work collaboratively in a multidisciplinary team.

Responsibilities

Assist in implementing treatment plans designed by Physical Therapists.
Provide direct patient care, including therapeutic exercises, manual therapy, and modality applications.
Monitor and document patient progress and report observations to the Physical Therapist.
Educate patients and families on treatment protocols, exercises, and self-care techniques.
Document patient treatments, progress, and responses in accordance with facility policies and regulatory standards.
Collaborate with Physical Therapists, other healthcare professionals, and support staff to coordinate patient care and support effective treatment outcomes.
