About The Position

Concierge Home Care is hiring a Home Health Physical Therapist Assistant (PTA). This role offers flexibility, independence, and the ability to increase earnings while providing meaningful one-on-one care in patients' homes. The coverage area is Clermont / Lake County, FL, with territories designed to minimize drive time through clustered scheduling. The position utilizes a pay-per-visit model with consistent patient volume, mileage reimbursement, and a data plan stipend.

Requirements

• Active, unencumbered Florida Physical Therapist Assistant (PTA) license
• Valid driver’s license, reliable transportation, and auto insurance

Nice To Haves

• Home health experience preferred

Responsibilities

• Provide hands-on therapeutic care following the plan of care established by the Physical Therapist
• Assist patients in improving mobility, strength, and independence
• Collaborate with patients, caregivers, and interdisciplinary teams
• Monitor patient progress and communicate updates to the supervising PT
• Complete documentation using streamlined, AI-assisted tools
