About The Position

Ultimate Therapy is seeking a full-time Physical Therapist Assistant at Autumn Lake at Windsor in Windsor, Connecticut. The role delivers individualized treatment in a skilled nursing rehabilitation setting under the direction of a supervising Physical Therapist, helping residents improve everyday movement and achieve functional mobility goals.

Requirements

• Active Connecticut Physical Therapist Assistant license.
• Ability to follow established care plans and resident-specific precautions.
• Clear communication, sound observation, and accurate documentation.

Nice To Haves

• Skilled nursing or post-acute experience; new graduates are welcome to apply.

Responsibilities

• Carry out prescribed interventions that address residents' functional mobility goals.
• Coach residents through exercises for strength, balance, and activity tolerance.
• Practice walking, getting in and out of bed, and transferring between surfaces with appropriate assistance.
• Reinforce safe techniques for using prescribed mobility devices.
• Observe residents' progress, symptoms, and responses to treatment, and communicate relevant findings to the supervising Physical Therapist.
• Document care promptly.
• Partner with nursing and therapy colleagues to support consistent mobility routines.
