About The Position

HealthPro Heritage has a great Physical Therapist Assistant (PTA) opportunity at Quabog Rehabilitation and Skilled Care Center in West Brookfield, MA. This role is in a long-term care setting and offers the chance to treat a diverse caseload including orthopedic, neurological, cardiac, pulmonary, and stroke patients. The facility features a modern, spacious rehab gym with updated therapy equipment and treatment modalities. You will work alongside an experienced interdisciplinary team and contribute to helping patients return home through functional training and discharge planning. Therapy services are provided up to 7 days a week, allowing for a steady caseload and collaborative scheduling. The environment is supportive and patient-centered, focused on individualized treatment plans and maximizing patient outcomes. The location offers a beautiful view overlooking the West Brookfield Town Common with a welcoming, community-focused atmosphere.

Requirements

Associate’s degree in Physical Therapist Assisting from an accredited program.
Valid state licensure as a Physical Therapist Assistant, or license eligible
Strong assessment, diagnostic, and therapeutic abilities with excellent communication and interpersonal skills.
Proficient in using diagnostic tools, therapy equipment, relevant software, and electronic health record systems, with a proven ability to work collaboratively in a multidisciplinary team.

Responsibilities

Assist in implementing treatment plans designed by Physical Therapists.
Provide direct patient care including therapeutic exercises, manual therapy, and modality applications.
Monitor and document patient progress, report observations to the Physical Therapist, and educate patients and families on treatment protocols, exercises, and self-care techniques.
Accurately document patient treatments, progress, and responses in compliance with facility policies and regulatory standards.
Work closely with Physical Therapists, other healthcare professionals, and support staff to ensure coordinated patient care and effective treatment outcomes.
