About The Position
Community Care, Inc. is looking for a compassionate and dedicated PTA to provide home health care to our clients in the Westmoreland and Fayette County area. Requirements
- Current Physical Therapist Assistant (PTA) license in the state of Pennsylvania.
- Graduate of an accredited Physical Therapist Assistant program
- Minimum 1 year of clinical experience
- CPR certification (required)
- Reliable transportation and a valid driver's license
- A caring heart, strong independence, and a passion for patient care
Nice To Haves
- Home Health experience preferred
Responsibilities
- Provide physical therapy treatment plans as established by the supervising Physical Therapist
- Provide patient education and instruction in home exercise programs
- Monitor, document, and report patient progress and any changes in condition
- Support patient in achieving functional goals that enhance safety and independence
- Collaborate with a multidisciplinary team to ensure continuity of care
- Document visits and patient progress accurately and timely
