About The Position

Community Care, Inc. is looking for a compassionate and dedicated PTA to provide home health care to our clients in the Westmoreland and Fayette County area. Requirements

  • Current Physical Therapist Assistant (PTA) license in the state of Pennsylvania.
  • Graduate of an accredited Physical Therapist Assistant program
  • Minimum 1 year of clinical experience
  • CPR certification (required)
  • Reliable transportation and a valid driver's license
  • A caring heart, strong independence, and a passion for patient care

Nice To Haves

  • Home Health experience preferred

Responsibilities

  • Provide physical therapy treatment plans as established by the supervising Physical Therapist
  • Provide patient education and instruction in home exercise programs
  • Monitor, document, and report patient progress and any changes in condition
  • Support patient in achieving functional goals that enhance safety and independence
  • Collaborate with a multidisciplinary team to ensure continuity of care
  • Document visits and patient progress accurately and timely
