About The Position

Independence Rehab is seeking a compassionate PRN Physical Therapist Assistant (PTA) to support our therapy team at Park Forest Care Center. At present, we need 5-10 hours of patient care per week. In skilled nursing, small progress creates life-changing impact. Our goal is to help each patient regain mobility, confidence, and independence so they can safely transition to the next level of care. If you enjoy working with older adults, value flexibility, and want to be part of a supportive clinical team, this is a great PRN opportunity.

Requirements

  • Active Colorado PTA license in good standing.
  • Graduate of an accredited PTA program.
  • Interest in geriatric rehabilitation and functional outcomes.

Nice To Haves

  • SNF experience preferred (one year ideal), but not required.

Responsibilities

  • Provide hands-on, functional therapy to a geriatric population.
  • Gait and mobility retraining.
  • Transfer training across all levels of assistance.
  • Strengthening, balance, and fall prevention.
  • Supporting patients with medically complex conditions.
  • Collaborating with the supervising PT on care progression.
  • Working alongside nursing, OT, and speech to support whole-person recovery.
