About The Position
Pleasant Hill Post Acute is seeking a compassionate, motivated, and dependable Physical Therapist Assistant (PTA) to join our rehabilitation team on a PRN basis. The PTA works under the direction of the supervising Physical Therapist to provide high-quality rehabilitation services that promote mobility, independence, safety, and overall quality of life for our residents.
Requirements
- Associate Degree from an accredited Physical Therapist Assistant (PTA) program required.
- Current California Physical Therapist Assistant license required.
- Strong communication, organization, and teamwork skills.
- Dependable and flexible with the ability to work on an as-needed basis.
- Commitment to providing compassionate, resident-centered care.
Nice To Haves
- Skilled nursing or post-acute rehabilitation experience preferred.
Responsibilities
- Provide skilled physical therapy treatments and interventions as directed by the supervising Physical Therapist.
- Assist residents in achieving individualized mobility and functional goals.
- Complete timely, accurate, and compliant clinical documentation.
- Maintain a safe and clean treatment environment while following infection control and safety protocols.
- Communicate regularly with the supervising Physical Therapist regarding resident progress, goals, changes in condition, discharge planning, and care coordination.
- Collaborate with nursing and other members of the interdisciplinary team.
- Participate in Resident Care Conferences and rehabilitation meetings as needed.
- Assist with training and support of Restorative Aides.
- Educate residents, families, and caregivers regarding treatment plans, home exercise programs, safety techniques, fall prevention, and maintenance programs.
- Assist with recommendations for durable medical equipment and adaptive equipment to promote resident safety and independence.
- Provide staff education and in-service training as needed.
- Report equipment concerns and help ensure rehabilitation equipment remains safe and functional.
- Participate in quality assurance and performance improvement activities.
- Maintain strict confidentiality of resident and company information.
- Develop positive and professional relationships with residents, families, coworkers, and healthcare professionals.
