About The Position

Pleasant Hill Post Acute is seeking a compassionate, motivated, and dependable Physical Therapist Assistant (PTA) to join our rehabilitation team on a PRN basis. The PTA works under the direction of the supervising Physical Therapist to provide high-quality rehabilitation services that promote mobility, independence, safety, and overall quality of life for our residents.

Requirements

  • Associate Degree from an accredited Physical Therapist Assistant (PTA) program required.
  • Current California Physical Therapist Assistant license required.
  • Strong communication, organization, and teamwork skills.
  • Dependable and flexible with the ability to work on an as-needed basis.
  • Commitment to providing compassionate, resident-centered care.

Nice To Haves

  • Skilled nursing or post-acute rehabilitation experience preferred.

Responsibilities

  • Provide skilled physical therapy treatments and interventions as directed by the supervising Physical Therapist.
  • Assist residents in achieving individualized mobility and functional goals.
  • Complete timely, accurate, and compliant clinical documentation.
  • Maintain a safe and clean treatment environment while following infection control and safety protocols.
  • Communicate regularly with the supervising Physical Therapist regarding resident progress, goals, changes in condition, discharge planning, and care coordination.
  • Collaborate with nursing and other members of the interdisciplinary team.
  • Participate in Resident Care Conferences and rehabilitation meetings as needed.
  • Assist with training and support of Restorative Aides.
  • Educate residents, families, and caregivers regarding treatment plans, home exercise programs, safety techniques, fall prevention, and maintenance programs.
  • Assist with recommendations for durable medical equipment and adaptive equipment to promote resident safety and independence.
  • Provide staff education and in-service training as needed.
  • Report equipment concerns and help ensure rehabilitation equipment remains safe and functional.
  • Participate in quality assurance and performance improvement activities.
  • Maintain strict confidentiality of resident and company information.
  • Develop positive and professional relationships with residents, families, coworkers, and healthcare professionals.
