About The Position

HealthPro Heritage has a great opportunity for a Licensed Physical Therapist Assistant (LPTA). This is a part-time position with flexible hours, allowing for 10-29 hours per week, or PRN is also available. The role is located at Grafton County Nursing Home in North Haverhill, NH, within a 135-bed SNF setting that is 4-Star CMS Rated. This position offers the chance to gain hands-on experience in a skilled nursing/long-term care environment, working with a diverse geriatric population including post-acute and rehab patients. You will build meaningful relationships with long-term residents and strengthen clinical skills in mobility training, gait, balance, and therapeutic exercise. Collaboration with an interdisciplinary team (PTs, OTs, nursing, and rehab staff) is key. The role is within a structured, county-run facility known for its consistent processes and stability, potentially leading to reduced burnout compared to full-time roles. Requirements

Associate’s degree in Physical Therapist Assisting from an accredited program.
Valid state licensure as a Physical Therapist Assistant, or license eligible.
Strong assessment, diagnostic, and therapeutic abilities with excellent communication and interpersonal skills.
Proficient in using diagnostic tools, therapy equipment, relevant software, and electronic health record systems, with a proven ability to work collaboratively in a multidisciplinary team.

Responsibilities

Assist in implementing treatment plans designed by Physical Therapists.
Provide direct patient care including therapeutic exercises, manual therapy, and modality applications.
Monitor and document patient progress, report observations to the Physical Therapist, and educate patients and families on treatment protocols, exercises, and self-care techniques.
Accurately document patient treatments, progress, and responses in compliance with facility policies and regulatory standards.
Work closely with Physical Therapists, other healthcare professionals, and support staff to ensure coordinated patient care and effective treatment outcomes.
