This role offers flexibility, autonomy, and support from one of Connecticut's most respected nonprofit healthcare organizations. Opportunities are available throughout three territories: South, Central, and North Connecticut. The territory-based model helps minimize travel while allowing you to become a trusted rehabilitation provider within the communities you serve. As an In-Home Outpatient Physical Therapist Assistant, you will work under the direction of a licensed Physical Therapist to provide skilled outpatient rehabilitation services under Medicare Part B. You will help patients recovering from orthopedic procedures, neurological conditions, chronic illnesses, and age-related mobility challenges improve their function and safely remain active in their homes and communities.
