About The Position
Therapy Management Corporation is hiring a Physical Therapist Assistant to provide physical therapy services in patients’ homes to adult and geriatric patients. Working under the direction of a Physical Therapist, the PTA helps patients improve independence, mobility, safety, strength, balance, and quality of life through personalized, patient-centered treatment. The role offers the opportunity to follow patient progress over time and work in a collaborative, therapist-driven clinical environment that emphasizes quality outcomes, clinical autonomy, and continuity of care. The position involves partnering with interdisciplinary teams, caregivers, and nursing staff, maintaining accurate clinical documentation, monitoring progress, and adapting treatment to patients’ rehabilitation needs. Requirements
- Be a licensed Physical Therapist Assistant (PTA) or license eligible in the state of practice.
- Graduate from an accredited Physical Therapist Assistant program.
- Have a passion for patient-centered care and improving quality of life for older adults.

Responsibilities
- Provide personalized physical therapy under the direction of a Physical Therapist to improve patients’ mobility, strength, balance, and functional independence.
- Implement patient-centered treatment plans that support recovery, safety, pain management, and long-term rehabilitation goals.
- Deliver hands-on therapy interventions to enhance movement, endurance, and quality of life for adult and geriatric patients.
- Partner with interdisciplinary teams, caregivers, and nursing staff to support patient outcomes and continuity of care.
- Maintain accurate clinical documentation, monitor patient progress, and adjust treatment plans based on individual rehabilitation needs.
