About The Position
Physical Therapist Assistant team members assist with ensuring the health and well-being of individuals by providing comprehensive and quality therapy services. This position is responsible for administering skilled physical therapy treatments as developed and recommended by a Registered Physical Therapist, ensuring that the highest degree of quality care is maintained at all times. Located in the heart of Williamsville, New York, Elderwood at Williamsville is a 200-bed skilled nursing and rehabilitation community providing short-term rehabilitation, skilled nursing care, memory care, respite services, and specialized ventilator care. The facility is committed to a patient-centered approach that focuses on restoring function, promoting independence, and enhancing quality of life through collaborative interdisciplinary care. As a member of our rehabilitation team, you'll work alongside experienced therapists, nursing professionals, and support staff who share a passion for delivering exceptional outcomes and compassionate care.
Requirements
- Graduate of an accredited Physical Therapist Assistant program
- Current PTA licensure in New York State (or eligibility for licensure)
- Strong clinical, communication, and documentation skills
- Ability to work effectively within an interdisciplinary team
- Graduation from a physical therapy assistant approved program required.
- New York State license to practice required.
- Current, vaild CPR/BLS credentials
- This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.

- Skilled nursing, rehabilitation, or long-term care experience preferred but not required
Responsibilities
- Administer physical therapy treatments and interventions as established by the Registered Physical Therapist
- Assist residents in achieving maximum physical function and independence
- Monitor resident response to treatment and communicate progress to the supervising therapist
- Accurately document treatments, progress notes, and resident outcomes
- Educate residents and families on therapeutic exercises and mobility techniques
- Collaborate with interdisciplinary team members to support individualized care plans
- Maintain compliance with facility, state, federal, and professional standards
- Promote a safe, positive, and motivating rehabilitation environment
- Assist the Physical Therapist in planning, developing and carrying out treatment programs and procedures for residents.
- Writes observations of resident’s behavior and progress in Progress Notes.
- Maintains statistical records and other departmental reports as requested by Physical Therapist.
- Assist with in-service training programs upon request.
- Assist with keeping supplies in good order and daily sanitation of equipment.
- Arrives to work on time, regularly, and working as scheduled; responsible for own transportation.
- Utilizes electronic timekeeping system.
- Observes and practices confidentiality of resident information and privacy when performing job duties.
- Recognizes and follows the dress code of the facility including wearing name tag at all times.
- Performs assigned duties according to the service excellence standards and goals established for this position, and for all staff members of this facility.
