About The Position

Requirements

Graduate of an accredited Physical Therapist Assistant (PTA) program
Active PTA license in the state of practice
Ability to work independently while supporting a multidisciplinary healthcare team
Ability to perform essential job functions with or without reasonable accommodation

Nice To Haves

Experience in SNF, Assisted Living, long-term care, or post-acute rehabilitation preferred
Strong communication and clinical collaboration skills

Responsibilities

Provide physical therapy treatments in Skilled Nursing Facilities (SNF) and Assisted Living (ALF) settings under the direction of a licensed Physical Therapist
Deliver evidence-based geriatric rehabilitation and post-acute care services
Implement individualized treatment plans focused on mobility, strength, balance, and functional independence
Document daily treatments and patient progress in compliance with Medicare, PDPM, and state Physical Therapy regulations
Participate in patient care conferences and interdisciplinary rehabilitation meetings
Communicate effectively with supervising Physical Therapists, Rehab Directors, nursing staff, and care teams
Support quality outcomes in long-term care and post-acute rehabilitation environments
