About The Position
Sunrise Home Health Services is seeking compassionate, motivated Physical Therapist Assistants (PTAs) to join their growing team in the Northwest Phoenix Valley. This role offers flexibility, independence, supportive leadership, and meaningful patient relationships. Sunrise provides the support, technology, and clinical resources to help clinicians succeed, whether they are experienced home health clinicians or transitioning into home health. Sunrise Home Health Services is one of Arizona's leading home health providers, delivering exceptional skilled care throughout the Valley. Their therapists help patients recover from joint replacement surgery, orthopedic injuries, stroke and neurological conditions, falls and balance disorders, general weakness and deconditioning, and chronic medical conditions affecting mobility. Every visit helps patients remain independent while reducing hospitalizations and improving quality of life.
Requirements
- Graduate of an accredited Physical Therapist Assistant program
- Current Arizona PTA license
- CPR Certification (or ability to obtain)
- Valid Arizona driver's license
- Reliable transportation and current auto insurance
- Must be a graduate from an accredited Physical Therapy program and licensed by the state of Arizona.
Nice To Haves
- Home health experience is preferred but not required.
- New graduates are encouraged to apply!
Responsibilities
- Provide individualized physical therapy treatments in patients' homes
- Implement treatment plans focused on improving mobility, strength, balance, endurance, and independence
- Monitor progress and communicate changes with the supervising Physical Therapist
- Educate patients and caregivers on home exercise programs and fall prevention
- Recommend assistive devices and home safety modifications
- Complete accurate documentation in accordance with Medicare and agency guidelines
- Collaborate with physicians, nurses, therapists, and other healthcare professionals to deliver outstanding patient care
