About The Position

HealthPro Heritage has a great opportunity for a Licensed Physical Therapist Assistant (LPTA) at The Heritage at Brentwood in Brentwood, TN. This full-time position is located in a beautiful upscale senior living community on a scenic 48-acre campus, offering a variety of clinical experience by treating short-term rehabilitation patients alongside long-term residents. This role allows you to grow your orthopedic, neurological, balance, and mobility skills within a collaborative interdisciplinary team. The setting includes Assisted Living, Skilled Nursing, Rehabilitation, and Memory Care, all in one location. Requirements

Associate’s degree in Physical Therapist Assisting from an accredited program.
Valid state licensure as a Physical Therapist Assistant, or license eligible.
Strong assessment, diagnostic, and therapeutic abilities with excellent communication and interpersonal skills.
Proficient in using diagnostic tools, therapy equipment, relevant software, and electronic health record systems, with a proven ability to work collaboratively in a multidisciplinary team.

Responsibilities

Assist in implementing treatment plans designed by Physical Therapists.
Provide direct patient care including therapeutic exercises, manual therapy, and modality applications.
Monitor and document patient progress, report observations to the Physical Therapist, and educate patients and families on treatment protocols, exercises, and self-care techniques.
Accurately document patient treatments, progress, and responses in compliance with facility policies and regulatory standards.
Work closely with Physical Therapists, other healthcare professionals, and support staff to ensure coordinated patient care and effective treatment outcomes.
