About The Position

FeldCare Connects is seeking a self-motivated Physical Therapist Assistant to provide home health care to adults of all ages in the North Port area. The role focuses on promoting patients’ ability to move, reducing pain, restoring function, and preventing disability. Requirements

• Physical Therapist license and state registration.
• Completion of a Physical Therapist curriculum approved by the American Physical Therapy Association, the Council on Medical Education and Hospitals of the American Medical Association, or the Council on Medical Education of the American Medical Association.
• Knowledge of physical therapy principles, theories, and methods in a rehabilitation environment.
• Skills in providing a variety of treatments.
• Effective written and oral communication skills.
• Ability to understand therapy in the context of patients’ ages, needs, and environments.
• Sound clinical reasoning in treatment planning, implementation, and monitoring patient progress.
• Ability to work with patients of all ages.

Responsibilities

• Assess and evaluate patient care needs.
• Provide treatment interventions intended to improve and enhance patients’ well-being, mobility, and function, reduce pain, and prevent disability.
• Monitor patient progress.
• Recognize when protocols or treatment plans may need modification and report this to the therapist.
• Prepare and maintain accurate and discreet records.
