About The Position

FeldCare Connects is seeking a self-motivated Physical Therapy Assistant to provide home health care to adults of all ages in the Healdsburg area. The role focuses on patients’ mobility, pain reduction, restoration of function, and prevention of disability. Following the plan of care based on the Physical Therapist’s assessment and evaluation, the assistant provides interventions, supports patient wellbeing, and evaluates progress.

FeldCare Connects partners with home health agencies and provides therapists with software and administrative resources to support efficient patient care. The position is offered as an independent contractor role, with flexibility to set a schedule and work in a preferred area.

Requirements

• Physical Therapy Assistant license and registration by the state.
• Completion of a Physical Therapy Assistant curriculum approved by The American Physical Therapy Association, The Council on Medical Education and Hospitals of the American Medical Association, or The Council on Medical Education of the American Medical Association.
• Ability and enthusiasm to work with people of all ages.

Responsibilities

• Provide services permitted by state laws governing Physical Therapy Assistants, following the plan of care and coordinating with other members of the health care team.
• Help patients use therapeutic, creative, and self-care activities to improve functioning and safety.
• Observe, record, and report patients’ responses to treatment and changes in their conditions.
• Instruct patients, families, caregivers, and health care team members in therapy activities they can participate in to assist the patient.
• Perform interventions aimed at improving and enhancing patient wellbeing, and evaluate patient progress.
