About The Position

Diablo Valley Post Acute is seeking a dedicated and compassionate Physical Therapist Assistant (PTA) to join our rehabilitation team on a PRN weekend basis. The Staff Physical Therapist Assistant assists the supervising Physical Therapist with resident-related activities and direct resident care. The PTA plays a vital role in helping residents improve mobility, restore function, and achieve their highest level of independence while delivering exceptional rehabilitative care in a skilled nursing setting.

Requirements

  • Current Physical Therapist Assistant (PTA) license in the State of California.
  • Graduate of an accredited Physical Therapist Assistant program.
  • Strong communication, organizational, and interpersonal skills.
  • Ability to work independently while collaborating effectively with an interdisciplinary team.
  • Commitment to providing compassionate, resident-centered care.

Nice To Haves

  • Skilled nursing, post-acute, or rehabilitation experience preferred.

Responsibilities

  • Provide resident treatments and therapeutic interventions as directed by the supervising Physical Therapist.
  • Complete timely, accurate, and compliant documentation in accordance with facility policies, governmental regulations, and third-party payer requirements.
  • Maintain a safe treatment environment by following infection control, safety, and equipment maintenance protocols.
  • Participate in Resident Care Conferences and Rehabilitation Department meetings as needed.
  • For California PTAs, maintain compliance with California Laws and Regulations related to Physical Rehabilitation Manual Article 4 §1398.44.
  • Assist the nursing department with training and education of Restorative Aides.
  • Communicate regularly with the supervising Physical Therapist regarding resident progress, goals, physician updates, discharge planning, and interdisciplinary care coordination.
  • Provide in-service training and educational programs for facility staff as needed.
  • Educate residents, families, and caregivers on treatment goals, home exercise programs, safety techniques, maintenance programs, and fall prevention strategies.
  • Recommend and facilitate ordering of durable medical equipment (DME) to promote resident independence and safety, including walkers, canes, splints, grab bars, and elevated toilet seats.
  • Maintain strict confidentiality of resident and company information.
  • Report equipment concerns promptly to ensure safe and effective operation.
  • Participate in quality assurance, performance improvement initiatives, and rehabilitation program development.
  • Build positive and professional relationships with residents, families, staff, and healthcare professionals.
