About The Position

FOX Rehabilitation is seeking licensed Physical Therapist Assistants to provide telehealth care to older adults in New Jersey. Requirements

Valid Physical Therapist Assistant license in the state(s) of practice, or eligibility to obtain one.
Degree from an accredited Physical Therapist Assistant program.
Telehealth experience.
Proficiency with computer skills and Microsoft Office, including Excel, PowerPoint, and Word.
Current CPR certification.
Integrity, compassion, and enthusiasm.
Ability to meet the stated physical demands of in-home and community-based care, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly; performing CPR; providing moderate to maximum assistance with patient transfers and bed mobility; repositioning patients; standing and walking for extended periods; climbing stairs; performing therapeutic activities; driving to care locations; and wearing PPE when required.

Responsibilities

Provide one-on-one physical therapy to older adults via telehealth, supplementing the virtual caseload with in-person house calls, senior living community visits, and other patient-care tasks as directed by leadership.
Implement individualized plans of care developed by the supervising Physical Therapist and deliver evidence-based treatment.
Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence.
Coordinate with the Supervising PT to complete and submit clinical and administrative documentation accurately and within required timeframes, following documentation procedures, intake and tracking processes, payer requirements, and audit standards.
Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
Meet the physical demands of in-home and community-based care, including patient transfers and bed mobility, repositioning, therapeutic activities, and travel to patient homes and senior living communities.
Perform CPR and wear PPE when required.
