About The Position

Putnam Nursing & Rehabilitation Center, a 160-bed skilled nursing home and rehabilitation facility in Putnam County, New York, is dedicated to providing high-quality care with respect and compassion. The Physical Therapist Assistant (PTA) is responsible for organizing and conducting physical therapy programs, planned and overseen by a Physical Therapist, to facilitate the rehabilitation of residents who have experienced a decrease in function. The PTA will assist in planning, organizing, developing, and implementing Physical Therapy services under the supervision of the Physical Therapist to ensure the highest degree of quality resident care.

Requirements

  • Graduate of an accredited school of Physical Therapy Assistant, PTA.
  • Licensed in NY State as a Physical Therapist Assistant, PTA; license in good standing.
  • Work well under pressure & willing to work harmoniously with all staff.
  • Sincere desire to work with the elderly, chronically ill, and handicapped.
  • Excellent customer service skills.
  • Must be able to communicate well verbally with residents, families and staff.
  • Treat residents and fellow staff with respect, dignity, and compassion.

Nice To Haves

  • Previous skilled nursing facility (SNF) experience highly preferred.

Responsibilities

  • Performs treatment programs for assigned residents.
  • Enters billing information in an accurate and timely manner.
  • Reports resident progress in meetings, such as UR Meeting and Patient/Family Conference, as deemed required or appropriate.
  • Consults with other members of the rehabilitation team to coordinate physical therapy with other therapeutic activities.
  • Provides residents with equipment as appropriate.
  • Charts notes in an informative and descriptive manner that reflects the therapy provided to the residents, as well as the resident’s response to the therapy.
  • Performs routine documentation as required and in accordance with established charting and documentation policies and procedures.
  • Signs and dates all entries made in residents’ medical record.
  • Cooperates with all departments and staff facilitating harmonious working conditions to promote optimal resident care.
