About The Position
Concept Rehab, a therapist-owned and operated rehabilitation company with over 40 years of experience, is seeking a PRN Physical Therapist (PT) to join their team in Lima, Ohio. This position offers scheduling flexibility, competitive compensation, and the opportunity to work alongside experienced rehabilitation professionals committed to patient-centered care. The role involves evaluating and treating patients with mobility, strength, balance, functional, and positioning challenges, developing individualized treatment plans to maximize independence and quality of life, and supervising Physical Therapist Assistants.
Requirements
- Graduate of an accredited Physical Therapy program.
- Current Physical Therapist license in Ohio.
- Ability to meet all state and federal professional requirements.
- Strong clinical judgment and patient care skills.
- Excellent communication and relationship-building abilities.
- Ability to work independently while collaborating with an interdisciplinary team.
- Professionalism, integrity, and accountability.
- Strong documentation and organizational skills.
- Commitment to ongoing professional development.
Nice To Haves
- Experience in skilled nursing, post-acute care, rehabilitation, or geriatric settings.
- Current CPR certification.
Responsibilities
- Evaluate patients for physical therapy services.
- Develop and implement individualized treatment plans.
- Provide skilled therapy interventions based on established plans of care.
- Supervise Physical Therapist Assistants in accordance with state practice regulations.
- Communicate effectively with physicians, nursing staff, families, and interdisciplinary team members.
- Participate in care planning and discharge planning activities.
- Document evaluations, progress notes, treatment notes, and discharge summaries accurately and timely.
- Educate patients, families, and facility staff on restorative and maintenance programs.
- Conduct in-service education and training for facility personnel.
- Maintain productivity and clinical quality standards.
