About The Position

This role involves providing specialized physiatry care in Skilled Nursing Facilities, focusing on patients experiencing pain or functional limitations that hinder their participation in therapy. The physiatrist will round at two facilities, five days a week, and will be the sole physiatrist on-site, requiring a high level of autonomy. The position is ideal for a motivated clinician passionate about improving rehabilitation outcomes and comfortable working independently within a collaborative clinical team.

Requirements

• Current, unrestricted Physician Assistant (PA) or Nurse Practitioner (NP) license.
• DEA licensed or willing to obtain DEA licensure.
• Comfort and interest in performing peripheral joint injections (knees, shoulders, hips).
• A genuine passion for providing professional, personalized care in an autonomous environment.
• Strong communication skills, with the ability to collaborate with multidisciplinary teams.
• Familiarity with EHR charting.

Nice To Haves

• Nursing home and/or physiatry experience is a plus, but not required.

Responsibilities

• Rounding at 2 Skilled Nursing Facilities, 5 days per week (2 days at one location, 3 days at the other).
• Delivering specialized physiatry care to 25+ patients per day who are experiencing difficulty participating in therapy due to pain or functional limitations.
• Performing focused assessments and developing individualized treatment plans in collaboration with facility primary care teams to optimize therapy participation and functional recovery.
• Providing targeted interventions such as joint injections, Botox treatments, dry needling, and medication management based on patient-specific needs.
• Serving as the sole physiatrist on-site, operating with a high level of autonomy.
• Coordinating care and referring to other specialties or services for issues beyond the scope of physiatry.
