About The Position

This is a part-time, 1099 independent contractor engagement for a Physiatry Advanced Practice Provider at a partner skilled nursing facility in Savannah. The role involves rounding on residents who are experiencing barriers to physical therapy due to pain or other conditions. The provider will assess, treat, and manage these issues to help residents progress in their recovery and return to therapy. This position offers the opportunity to own a panel, perform procedures, and work independently within a collaborative, mission-driven team.

Requirements

  • Current Nurse Practitioner or Physician Assistant license in the state of Georgia.
  • For NPs: certification in Internal Medicine, Geriatrics, or Family Practice required.
  • For PAs: current NCCPA certification required.
  • Must have an unrestricted DEA license or be willing to obtain one (reimbursed).
  • Familiarity with EHR systems and clinical documentation best practices.
  • Ability to work independently in a facility setting with physician support.

Nice To Haves

  • Minimum 1–2 years of NP or PA experience preferred.
  • Experience in subacute rehab, post-acute, SNF, or geriatrics strongly preferred.
  • Interest in or experience with procedures like peripheral joint injections and spasticity management.
  • Interest in geriatric and post-acute medicine.
  • Comfortable making decisions independently and knowing when to seek support.

Responsibilities

  • Evaluate residents flagged by therapy or nursing as unable to progress, performing focused functional, musculoskeletal, and neuromuscular exams to identify barriers.
  • Treat identified barriers directly, including peripheral joint and soft-tissue injections (knees, shoulders, hips), spasticity management (including botulinum toxin), trigger point and dry needling, and targeted medication management.
  • Collaborate with PT/OT, nursing, and the attending team to build a plan that includes DME, bracing, and adaptive equipment to ensure functional gains are sustained.
  • Follow through with residents by rechecking, adjusting treatments, documenting care, and escalating or referring issues outside the scope of physiatry.
  • Serve as the facility's go-to resource on therapy readiness, mobility, fall prevention, and pain management impacting recovery.
