About The Position

This role is for a Physiatry Nurse Practitioner Lead who will provide regional oversight of clinical care and facility relationships. The position involves rounding 2-3 days a week at a designated facility to care for patients experiencing pain or functional limitations that hinder therapy participation. The ideal candidate is a self-motivated and confident clinician passionate about improving rehabilitation outcomes and comfortable working independently. The role requires regular travel within the assigned region, from San Jose to Oakland weekly, and to Sacramento monthly, with compensated mileage. Physician Assistants are also encouraged to apply.

Requirements

  • 5+ years as a Nurse Practitioner (NP) or Physician Assistant (PA)
  • Current, unrestricted license as a Nurse Practitioner (NP) or Physician Assistant (PA)
  • DEA licensed or willing to obtain DEA licensure
  • Skilled Nursing Facility experience
  • Physiatry, Pain Management or Physical Medicine & Rehabilitation experience
  • Comfort in performing ultrasound guided peripheral joint injections (knees, shoulders, hips)
  • Genuine passion for providing professional, personalized care in an autonomous environment
  • Strong communication skills
  • Experience in a clinical leadership or supervisory role
  • Willingness to travel regularly within the assigned region

Responsibilities

  • Provide regional oversight of clinical care and facility relationships.
  • Round 2-3 days a week at a designated facility to care for patients experiencing difficulty participating in therapy due to pain or functional limitations.
  • Serve as the clinical point of contact for all Providers in the region.
  • Provide clinical training to ensure all Providers display excellent clinical decision-making and bedside manner.
  • Conduct time shadowing each Provider in the region.
  • Attend regular clinical and facility meetings with facility staff.
  • Perform focused assessments and develop individualized treatment plans in collaboration with facility primary care teams to optimize therapy participation and functional recovery.
  • Provide targeted interventions such as joint injections, Botox treatments, dry needling, and medication management.
  • Support physiatrists on-site and operate with a high level of autonomy.
  • Coordinate care and refer to other specialties or services for issues beyond the scope of physiatry.
  • Build relationships and value propositions with key stakeholders.
  • Collaborate with multidisciplinary teams.
