About The Position

This role is for a Physiatry Nurse Practitioner Lead who will be primarily responsible for regional oversight of clinical care and facility relationships. The position involves rounding 2-3 days a week at a designated facility to care for patients experiencing difficulty with therapy due to pain or functional limitations. The ideal candidate is a self-motivated and confident clinician passionate about improving rehabilitation outcomes and comfortable working independently. The role requires travel within the assigned region, from San Jose to Oakland weekly, and Sacramento monthly, with compensated mileage.

Requirements

  • 5+ years as a Nurse Practitioner (NP) or Physician Assistant (PA)
  • Current, unrestricted license as a Nurse Practitioner (NP) or Physician Assistant (PA)
  • DEA licensed or willing to obtain DEA licensure
  • Skilled Nursing Facility experience
  • Physiatry, Pain Management or Physical Medicine & Rehabilitation experience
  • Comfort in performing ultrasound guided peripheral joint injections (knees, shoulders, hips)
  • Experience in a clinical leadership or supervisory role
  • Willingness to travel regularly within the assigned region

Responsibilities

  • Regional oversight of clinical care and facility relationships
  • Rounding 2-3 days a week at a designated facility to care for patients
  • Serving as the clinical point of contact for all Providers in the region
  • Providing clinical training to ensure Providers display excellent clinical decision making and bedside manner
  • Shadowing each Provider in the region
  • Conducting regular clinical and facility meetings with facility staff
  • Performing focused assessments and developing individualized treatment plans in collaboration with facility primary care teams to optimize therapy participation and functional recovery
  • Providing targeted interventions such as joint injections, Botox treatments, dry needling, and medication management
  • Supporting physiatrists on-site and operating with a high level of autonomy
  • Coordinating care and referring to other specialties or services
  • Building relationships and value propositions with key stakeholders
