About The Position

Under the supervision of the Pharmacy Department, the Medication Reconciliation Technician is primarily responsible to obtain and document an accurate patient medication history profile that supports the mission, values and philosophy of Pharmacy Services and Sharp HealthCare.

Requirements

  • 2 Years experience in a pharmacy or similar environment.
  • California Pharmacy Technician License (TCH) - CA State Board of Pharmacy -REQUIRED
  • Must be able to demonstrate knowledge of medication nomenclature and dosing.

Nice To Haves

  • Combination of both inpatient and outpatient experience preferred, particularly outpatient experience.
  • Certified Pharmacy Technician (CPhT) - Pharmacy Technician Certification Board -PREFERRED

  • Ensures that patient's medication reconciliation gets completed in a timely manner.
  • Assist in ongoing development of process improvement to improve throughput.
  • Comply with all related state and federally mandated requirements in performing the job assignment.
  • All interviews of patients will be conducted in accordance with State and Federal Regulations.
  • All medical consultations will be referred to Medical Staff, Nursing Staff or Pharmacist staff to complete.
  • Demonstrates awareness of medication safety communications delivered by pharmacy management and safe medication practice pharmacist.
  • Qualitative work demonstrates thoughtful and thorough identification, evaluation and analysis of medication safety events.
  • Quantitative work reflects the desire to contribute to the ADE monitoring aspect of Safe Medication Practice.
  • Completes appropriate number of medication reconciliation profiles during shift.
  • Completes task within service level expectations with accuracy.
  • Assures that incoming phone calls are answered promptly and with courtesy.
  • Helps to resolve discrepancies in a timely manner and seeks assistance when additional resources are needed.
  • Assures that they do an appropriate number of medication reconciliation per day.
  • Capture and record patient electronic information (i.e. home medication name, dose, route, frequency, last dose date and time, allergies, vaccination history, patient weight, pregnancy and lactation status) in accordance with recognized standards of practice and hospital policy and procedures, within expected time frames to ensure a smooth, accurate and efficient medication reconciliation process.
  • Utilizes appropriate interpersonal and communication skills to interview patients in varying clinical settings to develop an accurate medication history profile.
  • Utilizes professional knowledge and expertise to validate patient medication information and document history in the electronic medical record.
  • Seeks additional resources to include patient family, primary care physician, or local pharmacy to validate and complete the patient medication history profile.
  • Assures incoming phone calls are answered appropriately, stating name, position and department.
  • Documents patient medication history in accordance with approved standards and guidelines to ensure information is complete and maintained appropriately to ensure integrity.
  • Resolves and/or troubleshoots issues in a timely manner and seeks additional resources as needed.
