The Medication History Pharmacy Technician is an essential member of the Department of Pharmacy and plays a critical role in supporting accurate and safe medication use for patients across the health system. This position is responsible for obtaining best‑possible medication histories for Emergency Department patients and admitted inpatients, with a focus on high‑risk populations. Working both independently and collaboratively, the technician partners closely with pharmacists, nurses, and providers to ensure clear communication and timely updates to the medication profile. The technician conducts structured patient and caregiver interviews, utilizes standardized tools and processes, and reviews external sources of truth to verify and document complete and accurate medication histories in the electronic health record. This role directly supports the medication reconciliation process by improving the accuracy of prior to admission medication lists, reducing discrepancies, and enhancing interdisciplinary medication management. The technician demonstrates proficiency with automation systems, the EHR, and office software, and maintains all required documentation in accordance with laws, regulations, and departmental policies. Through patient‑centered communication and exceptional customer service, the Medication History Pharmacy Technician contributes to optimal patient outcomes in the acute care setting.
