About The Position

ChristianaCare is seeking a skilled and motivated Pharmacy Technician II to support its growing ambulatory pharmacy services, including anticoagulation, primary care, and weight management programs. This role operates in a hybrid, centralized model, working closely with clinical pharmacists and a collaborative support team to enhance patient access, coordination, and continuity of care. This position may work remotely up to two (2) days per week. Working collaboratively with Clinical Pharmacy Specialists, Clinical Pharmacists, a Medical Technician, and an Access Coordinator, the Pharmacy Technician II will help optimize patient outcomes by managing workflows, removing barriers to care, and supporting high-quality, patient-centered services. The Pharmacy Technician II serves as a central point of coordination for ambulatory pharmacy services, managing medication access, adherence monitoring, and longitudinal follow-up. This individual triages patient needs, supports care coordination, and ensures seamless service delivery while operating with a moderate degree of autonomy.

Requirements

  • High school diploma or equivalent required
  • Pharmacy Technician Certification Board (PTCB) certification required
  • At least 3 years of pharmacy technician experience in retail, specialty, ambulatory care, or managed care setting

Nice To Haves

  • Strong experience with prior authorizations and appeals preferred
  • Experience in a clinic setting or team-based care model with pharmacists preferred

Responsibilities

  • Act as a centralized point of contact for patients and caregivers across assigned clinical pharmacy programs
  • Triage patient needs (e.g., lab coordination, medication access, refills) and escalate clinical concerns appropriately
  • Conduct proactive patient outreach to support therapy initiation, adherence, follow-up, and continuity of care
  • Facilitate primary care visits by collecting medication histories and ensuring accurate EMR documentation
  • Support Anticoagulation Center workflows, including INR monitoring, follow-up coordination, and results routing
  • Provide coverage for anticoagulation workflows in the absence of the Medical Technician
  • Assist with patient onboarding, engagement, and transitions of care, including scheduling support
  • Manage medication access processes, including prior authorizations, reauthorizations, benefit investigation, step therapy, and appeals
  • Identify and resolve barriers to medication access to avoid delays or interruptions in therapy
  • Partner with Primary Care pharmacists on population health initiatives and quality metrics
  • Collaborate closely with care team members to ensure efficient workflows, clear handoffs, and role clarity
  • Maintain accurate and timely EMR documentation, worklists, and tracking of patient care issues
  • Participate in process improvement initiatives to enhance operational efficiency and patient outcomes
  • Practice within scope and escalate clinical decisions to pharmacists as appropriate
