About The Position

Preferred Primary Care Physicians is seeking a full-time Pharmacy Technician – Care Coordinator to support our Ambulatory Care Providers in the Value-Based Care Department at our central business office in Canonsburg, PA. After training and director's approval, the candidate will have flexibility and an opportunity for a hybrid position, including an on-site and remote work schedule (Monday-Friday). This position will provide support to our ambulatory pharmacist, including interacting with and assisting patients and providers with medication-related issues. This position will collaborate with the ambulatory pharmacist on both PPCP and pharmacy payer-driven programs and other tasks. The Care Coordinator will also perform administrative tasks such as coordination of patient scheduling for PPCP's dietitians and Diabetic Education Classes (DSMES), distributing patient education, and other minor administrative tasks. The ideal candidate for this position will have an experienced Pharmacy Technician background along with being able to take on administrative duties including scheduling.

Requirements

  • Knowledge of pharmacy procedures and terminology
  • Excellent customer service and communication skills
  • Experience in using electronic health records or other healthcare related platforms
  • Ability to use applicable Microsoft products
  • Strong attention to detail
  • Ability to work independently
  • Registered Pharmacy Technician (Pennsylvania) required
  • At least 2 years previous pharmacy or healthcare experience required
  • High school diploma or equivalent

Nice To Haves

  • Previous administrative experience in a medical office and/or healthcare work environment preferred.
  • Certified Pharmacy Technician (CPhT) preferred

Responsibilities

  • Review referrals to dietitians and pharmacist
  • Manage and maintain patient scheduling on behalf of PPCP's dietitians and pharmacist
  • Perform administrative tasks on behalf of the care team as needed
  • Navigate medication affordability concerns directly with patients and providers
  • Assist PPCP's pharmacist with addressing formulary changes, managing pharmacy-related STARs measure data, and developing/maintaining workflows for pharmacy initiatives
  • Maintain thorough and proper documentation within the electronic medical record
  • Other duties as assigned
