Pharmacy Medication History Assistant
About The Position
Emplify Health by Gundersen is seeking a Pharmacy Medication History Assistant to join their hospital pharmacy team in La Crosse, WI. This role is ideal for students in healthcare pre-professional programs, such as pre-pharmacy, who want hands-on hospital pharmacy experience. The assistant, under pharmacist supervision, will complete accurate medication histories for admitted patients. This involves interviewing patients, families, and caregivers, verifying medication information with external sources, and entering reconciled medication lists into the EHR. The position acts as a pharmacist extender, supporting safe and accurate medication reconciliation in an inpatient setting.
Requirements
- High school diploma or equivalent
- Registration as a Pharmacy Technician in Wisconsin
- 1 year of post-high school education in a healthcare professional or pre-professional course of study, such as pre-pharmacy or pre-medicine, preferred
Responsibilities
- Interview patients, families, and caregivers to obtain medication information.
- Verify medication information with outside sources when needed.
- Enter reconciled medication lists into the electronic health record (EHR).
- Function as a pharmacist extender to support safe and accurate medication reconciliation.
