About The Position

SUMMARY: AxessPointe Community Health Center, Inc. (API) is a Federally Qualified Health Center serving the underserved in Northeast Ohio. API is committed to providing a quality, affordable and compassionate health home for every patient, every time, in the communities we serve. API is utilizing interdisciplinary services as an integral part of the medical home model. Our API Pharmacy is a 340b Pharmacy SUPERVISOR: Director of Pharmacy DUTIES AND RESPONSIBILITIES: Pharmacy Accurately and timely, fill all prescriptions. Review prescriptions for Drug-Drug Interactions, Food-Drug Interactions, Drug Allergies, Drug Misuse, etc. Maintain patient profiles and pharmacy records. Counsel patients on their medications. Supervise pharmacy technicians, pharmacy students, and pharmacy residents. Participate in 3rd party billing and inventory management operations. Oversee patient assistance program. Follow current and new Federal and State Laws, including 340b integrity regulations. Act as the pharmacist-in-charge. Establish and Provide Clinical Pharmacy Services Identify patients for clinical referral. Work in collaboration with all members of the care team to assess, facilitate, plan and advocate appropriate services to meet the patient’s health requirements on an individual basis. Provide Medication Therapy Management (MTM) and disease management services. Provide immunizations. Authorizing refills in accordance with the Clinic Refill Policy. Precept pharmacy residents and students. Maintain good working rapport with physicians and other health care professionals. Answer drug information questions posed by providers. Provide educational in-services to medical and dental providers Evaluate the delivery of patient care services through research and quality improvement initiatives. Participate in service development/improvement. Additional Tasks as Assigned by the Director of Pharmacy.

Requirements

  • Education, training and/or experience that demonstrate possession of the knowledge, skills and abilities stated above.
  • Doctorate Degree in Pharmacy from an accredited College of Pharmacy.
  • Pass background check.
  • Have and maintain personal professional insurance. (malpractice)
  • Ambulatory care or internal medicine residency or 5 years ambulatory care experience.
  • Certification to give immunizations.
  • License in Good Standing with the Ohio State Board of Pharmacy.
  • Have and maintain a current BLS/CPR certificate

  • While performing the duties of this job, the employee is frequently required to use close vision (clear vision at 20 inches or less). The employee will occasionally be working near moving mechanical parts, be exposed to outdoor weather conditions and exposed to a risk of electrical shock while running the copier, printer, other standard equipment and answering the telephone. The noise level in the work environment is low to moderate. Stress level can be high at times.
