The Pharmacy Clinical Practice Specialist provides comprehensive pharmaceutical care to patients on a given medical service. This role involves developing and maintaining a clinical practice within a patient care service, cooperating with medical and nursing personnel to optimize the pharmacotherapeutic aspects of patient care. The specialist is responsible for the optimal selection of pharmacologic agents and the design of regimens to maximize therapeutic benefit and minimize toxicity, ensuring therapy is appropriate for the patient’s diagnosis, weight, and age. Daily follow-up on the clinical effects of the regimen with adjustments as necessary to achieve desired outcomes is also a key function. Additionally, the role involves providing drug and clinical information to other health professionals, offering professional education on new drugs or technologies, and educating and counseling patients about their drug therapy. The specialist participates in clinical guideline development, critical pathways, disease management, and drug use programs, as well as providing clinical education and training to students, satellite pharmacists, residents, and fellows. Quality assurance programs and supporting operational components of the pharmacy are also part of the responsibilities. Marginal functions include adhering to internal controls and reporting structures, and performing related duties as required.
