About The Position
This role is at Ascension St. Vincent's Southside Hospital in Jacksonville, FL, within the Ascension Rx Specialty Pharmacy. The position is full-time with a rotating schedule. The pharmacist will play a crucial role in ensuring patient safety and optimizing patient outcomes through meticulous medication order reviews, collaboration with physicians, and patient education.
Requirements
- Licensed Pharmacist credentialed from the Florida Board of Pharmacy required. licensure or eligible.
- Bachelor's or advanced degree in Pharmacy required.
Responsibilities
- Assure patient safety and care quality by meticulously reviewing medication orders to identify, prevent, and resolve potential drug-related problems.
- Optimize patient outcomes by collaborating with physicians to recommend appropriate formulary alternatives and providing expert drug information to the medical team.
- Oversee and mentor pharmacy technical personnel in the preparation and accurate dispensing of medications, ensuring compliance with all regulatory standards.
- Educate and counsel patients on their medication therapies, promoting effective and safe use for better health outcomes.
- Facilitate seamless clinical operations by ensuring the appropriate selection, dosing, and dispensing of all medication orders.
