About The Position

Valence Care, a family of healthcare companies, is seeking a Licensed Pharmacist to join our team at Prime Care Pharmacy in Brooklyn. This is a part-time, temporary position expected to last approximately six months, with the potential for continued or expanded opportunities as our business grows. This position combines retail pharmacy and long-term care (LTC) responsibilities, with a focus on medication safety, accuracy, and quality patient care.

Requirements

  • Bachelor’s degree in Pharmacy or PharmD from an accredited pharmacy program.
  • Current, active New York State Pharmacist license and registration.
  • Strong knowledge of medications, drug interactions, and medication safety.
  • Excellent communication, organizational, and interpersonal skills.
  • Ability to work effectively with patients, caregivers, physicians, nurses, and other healthcare professionals.

Nice To Haves

  • Previous retail, clinical, home health, community, or healthcare experience preferred.
  • Experience with PrimeRx/Micro Merchant pharmacy software preferred.

Responsibilities

  • Review, interpret, and verify prescription orders for accuracy, appropriate dosing, drug interactions, contraindications, and other potential safety concerns.
  • Prepare and dispense medications in accordance with New York State and federal pharmacy regulations and company policies.
  • Provide professional pharmacy services and medication counseling to retail customers.
  • Educate patients and caregivers on proper medication use, dosing, administration, side effects, and adherence.
  • Review and oversee medication orders for long-term care facilities.
  • Verify blister packs/unit-dose packaging for accuracy, including medication, strength, dosage, quantity, labeling, and administration schedule.
  • Monitor patient medication profiles for drug interactions, duplications, allergies, and other potential concerns.
  • Communicate with physicians, nurses, caregivers, LTC facilities, and other healthcare professionals regarding prescriptions and medication therapy.
  • Resolve prescription discrepancies, refill requests, medication changes, and other medication-related concerns.
  • Maintain accurate pharmacy records and protect patient confidentiality.
  • Support day-to-day pharmacy operations, including inventory management, medication storage, and quality-control procedures.
