About The Position

Under the supervision of the Clinical Pharmacy Coordinator and the Pharmacist in charge, the Clinical/Staff Pharmacist is responsible for the planning, evaluation, supervision, coordination, implementation, and administration of comprehensive and effective quality Pharmacy programs (new & existing) at the clinic. This role involves working with other site Pharmacists on a regular basis, providing professional advice and consultation concerning drugs and their administration to patients/staff, maintaining a drug formulary, establishing and maintaining excellent patient/staff relations, maintaining licensure, reporting data, utilizing approved Clinical Pharmacy Protocols, and billing revenues from customers. The position will also include filling in at Campo Clinic as needed. This role is approximately a 50% Clinical Pharmacist and 50% Staff Pharmacist position.

Requirements

Successful completion of a graduate pharmacy degree, Doctorate in Pharmacy (Pharm.D.) from an accredited pharmacy school.
Two years of full-time, post-degree, professional experience in a public or private agency providing hospital or equivalent services.
Thorough knowledge of laws, rules, and regulations governing pharmacology, treatment, policies, and procedures relating to the provision of pharmacy management services.
Current California Registered Pharmacist license.
Valid California driver’s license.
Insurable under SIHC vehicle insurance policy.
Certifications and/or licenses appropriate to the positions required education and profession must also be valid and maintained.

Nextgen and QS1 experience highly preferred.

Responsibilities

Accurate and timely administration of prescriptions, reviewing drug orders for completeness and appropriateness, and consulting with physicians to correct any issues.
Reviewing patient history, current therapy, diagnosis, allergies, and recent drug history prior to dispensing medications.
Using generic brands whenever possible and providing labeled medications with potential drug interactions.
Recording prescriptions in the medical record and overseeing pharmacy data processing and billing.
Utilizing approved SIHC, Inc. Medication Management protocols (Refill protocol, Smoking Cessation Protocol, Anticoagulation protocol), performing medication reconciliation, and conducting patient education at SIHC, Inc. Health Fairs.
Providing professional, courteous, accurate, and timely information to patients, medical staff, and clinic personnel.
Supplying prescriptions to patients waiting in the lobby on a priority basis and logging prescription requests.
Controlling ordering, stocking, and distribution of all drugs within the clinic, including clerical and distributive functions.
Stocking and maintaining clinic “crash cart” and performing monthly pharmacy and nursing unit inspections to remove expired or unusable drugs.
Verifying proper storage conditions and security, and properly destroying or returning unusable drugs.
Reviewing therapy for potential drug therapy danger by calculating pharmacokinetic parameters for drug dosing.
Developing and maintaining a drug formulary in consultation with the Medical Director, and using generic drugs whenever possible.
Reviewing adverse drug reactions, logging incidents, and filing written reports.
Participating in cardio-pulmonary resuscitation and Code Blue activities.
Responding immediately to “Code Blue” calls.
Complying with all federal & state laws, regulations, and practices generally accepted in licensed Pharmacy operations.
Performing other clinical duties as assigned.
Responsible for the Pharmacy department’s opening/closing, efficient operations, timely billing of service fees, and submission of billing status reports.
Ordering pharmaceutical drugs & supplies from via the 340B Drug Pricing Program and enforcing all 340B Drug Pricing Program Federal Regulations.
Supervising personnel working under the direction of the Pharmacist, which may include a Pharmacy Technician and other temporary support staff.
Maintaining and reviewing records & reports for the Pharmacy program and providing for quality assurance.
Responsible for submission, monitoring, and reporting data to various end-users.
Attending monthly Medical Staff meetings when staffing allows.
Attending required professional educational meetings.
Responsible for maintenance and renewal of State of California Registered Pharmacist licensure.
Complying with all SIHC policies & procedures.
Ability to work a flexible schedule may be required based on need.
