About The Position

UPMC Williamsport is hiring a full-time Pharmacist to support safe, effective, and cost-efficient medication therapy. This position works day shift with evening rotations, with typical hours of 7:30 a.m. – 4:00 p.m., and includes every third weekend and two holidays per year. Pharmacists are responsible for supporting centralized and decentralized medication-use systems, collaborating with the healthcare team, and contributing to quality, safety, and regulatory initiatives that improve pharmacy practice and patient outcomes. The ideal candidate will possess prior hospital experience and active involvement in clinical rounding as a pharmacist. Final candidates will be selected for a job title within the career ladder that reflects level of education, experience, and manager discretion at time of offer.

Requirements

  • B.S in Pharmacy or Doctor of Pharmacy (PharmD) degree from an ACPE accredited College of Pharmacy
  • Current licensure as a Pharmacist in practicing state
  • Act 34

Responsibilities

  • Interpret, evaluate, and verify physician orders and prescriptions to prepare, calculate, and dispense medications in compliance with the Pennsylvania Pharmacy Practice Act, federal drug laws, and regulatory standards
  • Assess patient-specific parameters, incorporating age- and disease-specific considerations into drug therapy decisions and patient education
  • Review and maintain patient medication records to identify allergies, drug interactions, therapeutic duplications, and other medication-related concerns
  • Identify, document, and report suspected adverse drug reactions accurately and in a timely manner
  • Promote safe and effective formulary management by minimizing non-formulary use, applying therapeutic interchange protocols, and supporting rational drug therapy selection
  • Prepare and dispense medications for inpatient and outpatient use, including intravenous admixtures, using aseptic technique in approved sterile preparation areas
  • Communicate with physicians, nurses, and other healthcare professionals to clarify unclear, incomplete, or illegible medication orders
  • Serve as a medication resource and liaison to clinical teams, departments, and external partners
  • Supervise and support pharmacy technicians, pharmacy aides, student interns, and other ancillary staff involved in pharmaceutical care
  • Provide drug information, education, and training to healthcare professionals, pharmacy students, and pharmacy residents, serving as a preceptor as assigned
  • Utilize pharmacy computer systems, automation, and technology to enhance medication safety and workflow efficiency
  • Participate in quality improvement, medication safety, and regulatory compliance initiatives by monitoring processes, analyzing data, implementing interventions, and evaluating outcomes
  • Adhere to all hospital policies, professional practice standards, and quality management requirements in the delivery of pharmaceutical care
