About The Position

Capital Cardiology Associates (CCA) is seeking a motivated Clinical Pharmacist (PharmD) to join our team and become part of an innovative expanding service! CCA is a fast-paced cardiology practice that provides service to over 80,000 patients in the Capital Region and beyond. Our pharmacists are integrated into several of our clinical services. The primary focus of the position will be to provide clinical pharmacy services, optimizing medication outcomes with a focus on cardiometabolic care. Deliver direct patient care addressing weight management, cardiovascular disease, endocrine disorders, hyperlipidemia, and lifestyle medicine.

Requirements

  • Doctorate of Pharmacy Degree from an ACPE accredited school of Pharmacy
  • Successful completion of ASHP-accredited PGY1 clinical pharmacy residency
  • Current, active, Pharmacist License required at hire; NYS license required within 3 months of hire
  • National Provider Identifier required at hire or within 3 months of hire
  • Excellent communication and interpersonal skills
  • Leadership ability

  • Exercises discretion, objectivity and confidentiality in all aspects of daily routine
  • Able to multi-task, highly organized

Nice To Haves

  • PGY2 pharmacy residency or years of practice in direct patient care preferred

Responsibilities

  • Deliver evidence-based, patient-centered pharmacologic and lifestyle care plans, using approved protocols in collaboration with multidisciplinary team (physicians, APPs, dietitians, exercise physiologists, nurses), via in-person and telehealth visits
  • Optimize medication therapy by identifying and resolving drug-related problems
  • Monitor clinical outcomes, adjust therapies, and manage complications
  • Manage cardiometabolic risk factors and cardiovascular conditions (e.g., obesity, hypertension, hyperlipidemia, heart failure, smoking cessation) through developing, implementing and monitoring treatment plans using shared-decision making
  • Communicate and educate care plans, including medication, adherence and lifestyle modifications, effectively with patients
  • Provide clinical guidance and education to staff and trainees (students, residents)
  • Document care accurately and timely in the EMR, as well as related outcome platforms
  • Contribute to quality and operational improvements and clinical initiatives
  • Support team development, training, and a collaborative work environment
  • Promote a positive public image for the profession and for CCA
  • Assist with training of staff, contributes to an open learning environment
