About The Position

The Pharmacist-Per Diem position is a part-time role with a rotating shift. The primary responsibilities include accurate and timely medication dispensing, monitoring drug usage, training pharmacy staff and students, and ensuring compliance with all regulations and policies. The role also involves providing pharmaceutical care by evaluating physician orders, reviewing drug information, making therapeutic recommendations, counseling patients, and managing parenteral nutrition. Age-related patient care considerations for neonates, pediatrics, adults, and geriatrics are essential. The position requires adherence to hospital and government regulations regarding controlled substances, chemotherapy, and investigational drugs, as well as state and federal laws for prescription dispensing. Regular review of departmental and hospital policies and procedures is expected.

Requirements

  • Graduate School of Pharmacy with a minimum of a Bachelor's degree
  • Current Florida Pharmacy License

Nice To Haves

  • Pharm D is preferred
  • Previous experience as a Pharmacist in a hospital setting is preferred

Responsibilities

  • Input orders in a timely and consistent manner with workload demands.
  • Ensure all medications and intravenous solutions dispensed are accurate and complete.
  • Ensure all routinely-scheduled pharmaceuticals are available for administration at the scheduled time.
  • Ensure all dispensed pharmaceuticals are accurately weighed, measured, admixed, and labeled properly.
  • Monitor work progress of technicians and assess and assign work priorities.
  • Train pharmacy students, pharmacy interns, technicians, and peers in systems related to the pharmacy department.
  • Demonstrate working knowledge of phone, CRT, and printers.
  • Monitor drug usage as it relates to available supply and ensure adequate supplies are stocked/ordered.
  • Communicate effectively, both orally and in writing, in a specific and informative manner.
  • Prepare parenteral products using aseptic technique.
  • Ensure all patient charges and credits are current and accurate.
  • Report problems and concerns to the Supervisor in a timely and constructive fashion.
  • Evaluate physician orders accurately for input into the pharmacy computer system for all disease states and patient age groups.
  • Thoroughly review incompatibility data, drug interaction data, and drug allergy information for each order processed.
  • Provide clear and concise recommendations for therapeutic substitutions or optimizing drug therapy.
  • Present patient counseling and drug information in a knowledgeable and informative manner.
  • Ensure reported adverse drug reactions are accurate and thorough.
  • Make pharmacokinetic recommendations for optimum dosage adjustments resulting from effective patient evaluation and physician consultation.
  • Manage parenteral nutrition following procedural guidelines to formulate nutritional assessment and treatment goals.
  • Monitor patient drug therapy with regard to drug use evaluation criteria.
  • Document interventions in the pharmacy computer system.
  • Document events and near misses in the event report form and send to the Pharmacy office for review.
  • Obtain and interpret information in terms of patient's needs, demonstrating appropriate knowledge of growth and development.
  • Understand the range of treatment needed by all patients including neonates, pediatrics, adult, and geriatric.
  • Assess and implement age-related dosing protocols.
  • Complete nursing unit inspections monthly.
  • Ensure Quality Control documentation and logs are complete with signatures.
  • Ensure all controlled substance documentation is legible and complete in accordance with hospital and government regulations.
  • Strictly adhere to chemotherapy and investigational drug policy, procedure, and protocols.
  • Dispense prescriptions in accordance with State and Federal laws.
  • Review departmental and hospital policies and procedures annually to reinforce compliance and knowledge thereof.
  • Maintain current knowledge of the pharmacy profession as well as applicable laws and regulations.
