About The Position

The Pharmacist serves as a vital resource for medical providers, offering expertise in pharmacy practice, patient profiles, and prescription review. This role involves educating medical staff on proper pharmacy procedures, monitoring patient drug therapy for potential issues, and maintaining accurate patient medication records. The Pharmacist is responsible for documenting all work performed and ensuring compliance with guidelines from organizations such as The Joint Commission, DEA, FDA, and BOP. This position requires independent decision-making, a focus on efficient and economical pharmacy operations, and the careful selection, dispensing, and preservation of medications. The Pharmacist must be knowledgeable about drug differences, therapeutic efficacy, and proper storage to prevent deterioration. They also provide drug information, manage adverse drug reaction reporting, and consult with medical staff on medication-related aspects of patient treatment.

Requirements

Valid and current License that allows for the practice of Pharmacy (Verified).
Proof of CPR certification.

Responsibilities

Serves as a resource person to medical providers regarding pharmacy practice, patient profiles, and prescription review.
Conveys proper pharmacy procedures and practices to medical staff.
Monitors drug therapy for contraindications, interactions, allergies, and appropriateness.
Maintains patient medication records, including medication profile formats.
Documents all work performed.
Ensures familiarity with administrative guidelines of relevant healthcare organizations (Joint Commission, DEA, FDA, BOP).
Provides a balanced and efficient pharmacy service, establishing and maintaining service standards.
Maintains maximum efficiency in economy of operations.
Makes independent decisions and recommendations for improvements.
Responsible for the selection, dispensing, and preservation of drugs and therapeutic agents.
Identifies and ascertains differences between drugs related to therapeutic efficacy.
Periodically inspects drugs for adequacy, overstocking, arrangement, storage, proper rotation, and deterioration.
Assists in maintaining an inventory of commonly utilized drugs.
Assumes responsibility for the security of drugs in custody.
Uses proper techniques for drug storage (refrigeration, protection from heat/light).
Maintains awareness to avoid drug deterioration, loss of potency, formation of toxic products, or adverse patient care effects.
Provides a wide range of drug information to medical staff.
Maintains the adverse drug reaction (ADR) reporting system by investigating events.
Is available to medical staff for consultations regarding patient treatment aspects of medication, including drug compatibility, mode of action, drug reactions, and latest developments in pharmacy.
Reviews medication orders prescribed by medical staff through individual consultations and medical record reviews.
