In collaboration with patients, families (as defined by the patient) and staff across all disciplines and departments is responsible for providing pharmaceutical care services. This role ensures that prescriptions are dispensed safely and accurately, conducts patient drug regimen reviews, and provides patient prescription counseling. The pharmacist is also responsible for preparing and maintaining required records, communicating with patients and clinical staff, and performing other assigned duties. All tasks must comply with regulatory and accreditation standards, including The Joint Commission and CommUnityCare Standard Operating Procedures and Policies. The role emphasizes building favorable internal relationships, interacting respectfully with patients and families, and collaborating with the care team to provide patient-centered care. Professional conduct and adherence to company policies, mission, and values are essential. Mandatory training and maintaining credentials/licensure are also required.
