Provide medication management services to patients of Health Sciences North (HSN) including the evaluation of the appropriateness and effectiveness of patients’ medications, drug information, and interdisciplinary drug therapy management support and drug distribution services. The role involves optimizing drug therapy, reviewing orders for accuracy, and providing medication management services to the North East Cancer Centre (NECC) and HSNRx retail pharmacies. This includes working with patients, families, caregivers, and other health care professionals to provide services such as medication counselling and adherence support. The pharmacist will also assist with insurance approvals for complex outpatient medications, respond to drug/therapeutic information requests, provide follow-up recommendations to prescribers, and perform medication reconciliation. Ensuring compliance with the standards of practice of the Ontario College of Pharmacists (OCP) and HSN policies, including drug use control and security of narcotics, is crucial. The position also involves performing Therapeutic Drug Monitoring, educating healthcare providers and patients on drug therapy, advocating for adverse drug reaction and medication error reporting, and contributing to quality improvement projects aligned with HSN’s Strategic Plan. Additionally, the role includes supporting learner development through mentorship and preceptorship, participating in health and safety training, and representing the department on committees.
