About The Position

Sheridan Community Hospital is a vital healthcare resource in our rural community, providing compassionate, patient-centered care in a Critical Access Hospital setting. Our dedicated staff take pride in delivering high-quality services with a personal touch, ensuring our patients receive the care they need close to home. The pharmacist is responsible for execution of established aspects of the pharmaceutical needs of patients in both the inpatient and outpatient setting as developed by Pharmacy Director. The pharmacist shall be a resource to physicians and nurses in matters dealing with all aspects of drug therapy. It is also imperative that Pharmacist maintains compliance with all agencies that regulate the use of drugs and their preparation and distribution of medications.

Requirements

  • Bachelor of Science degree in Pharmacy from an accredited College of Pharmacy
  • Licensed by the State of Michigan as a Pharmacist
  • Three years of experience as a pharmacist, preferably as a hospital pharmacist.
  • Excellent written and verbal skills.
  • Ability to interact with physicians, nurses, and other health care professionals on matters that deal with the pharmaceutical needs of patients.

Responsibilities

  • Assuring that the business practices of their departments are following current state and federal law.
  • Implementing and coordinating all policies dealing with pharmaceuticals within the hospital established by Director of Pharmacy.
  • Acting as a leader for Antibiotic Stewardship Program and Antimicrobial Stewardship.
  • Responsible for procurement, storage, preparation, dispensing, recording, and control of all drugs used in the hospital.
  • Coordinating pharmacy policies that affect other departments.
  • Assisting in preparing and keeping current the hospital formulary.
  • Maintaining adequate supply of medications listed in the hospital formulary.
  • Preparing medications by reviewing and interpreting physician orders and detecting therapeutic incompatibilities.
  • Establishing relationships with other departments to be used as resources for Pharmacy related issues and emergencies.
  • Training all personnel assigned to the pharmacy.
  • Restocking medications in the night cupboard and crash carts throughout the hospital.
  • Overseeing and conducting inspections of all drug storage areas within the hospital for outdated medication on a weekly bases.
  • Maintaining current records of all controlled substances.
  • Investigating any discrepancy found during routine counts.
  • Procuring all IV solutions and is responsible for proper functioning of pumps used for administration of fluids.
  • Supervising and assisting in dispensing of prescriptions to the nursing units.
  • Informing physicians and nursing personnel of any possible untoward reactions to medications or drug interactions.
  • Ensuring admixtures are prepared according to established policy.
  • Responsible for proper disposal of any hazardous substance that is prepared in the pharmacy.
  • Preparing a policy for handling spills of hazardous or potentially hazardous materials used in the pharmacy.
  • Keeping the medical staff and nursing personnel informed of new trends in drug therapy.
  • Attending educational seminars to keep current on all aspects of pharmacology.
  • Being an active member of the Pharmacy and Therapeutics Committee.
  • Implementing decisions of the P+T Committee per direction of Pharmacy Director.
  • Establishing methods for promoting the safe administration and use of medications.
  • Participating in preparation of annual operational and capital budget with strategies and goals for the department, to the hospital administration.
  • When unavailable to the department during off duty hours, designating an alternate to deal with emergency situations.
  • Responding to any complaints by patients or families that relate to pharmacy issues.
  • Reporting any serious problems to the Risk Manager immediately.
  • Maintaining up to date pharmacy library.
  • Participating as necessary in the training of pharmacy interns and nursing students.
  • Interacting with the community on issues that promote health care concerns and the hospital.
  • Coordinating and providing for adequate coverage of the department during both on duty hours and off duty hours.
  • Participating in quality improvement projects.
  • Attending training and meetings as required.
  • Maintaining awareness of current quality and safety measures on the unit and following guidelines or reporting measures to ensure safety of patients, visitors, and staff.
  • Utilizing knowledge and skills in quality improvement and research methodologies.
  • Performing other related duties as assigned.
  • Active participation in hospital committees and task forces.
  • Active participation in quality management issues.
  • Submitting reports and data as requested by the Quality Management Council.
  • Demonstrating the ability to use channels of communication to address concerns, problems, and conflicts with other departments.
