The pharmacist is responsible for day-to-day comprehensive, patient-oriented pharmacy services during their shift across assigned department areas. Working under the general direction of an administrative or clinical coordinator, the pharmacist directs and supervises assigned support staff and uses sound management and clinical judgment when coordinators or more senior staff are unavailable. The pharmacist helps identify and resolve problems in their practice area and participates in training pharmacy interns and externs. The role coordinates dispensing of unit-dose medications, intravenous admixtures, investigational and research drugs, and other pharmaceuticals. It includes identifying patients’ medication needs; reviewing charts and gathering information from patients and care providers; assessing drug-related information for potential therapy problems; verifying medication orders and prepared medications; providing drug and poison information; conducting assigned monthly nursing-unit reviews; and monitoring patients’ responses to therapy, including providing pharmacokinetic dosing consultation when needed. The pharmacist also contributes to education and training for pharmacy students, residents, fellows, and support staff and assists with support-staff performance evaluations.
