About The Position

Park Nicollet is looking to hire a Pharmacist to join our Medication Therapy Management team! Come join us as a Partner for Good and help us make an impact on the care and experience that our patients and their families receive every day. The purpose of this position is to improve population based and patient specific medication related outcomes through collaboration with a multi-disciplinary medical team in the ambulatory setting. The pharmacist assumes a primary responsibility and accountability for the provision of safe and effective drug therapy and other medication management services intended to achieve outcomes related to the mitigation or prevention of disease, reduction of patients’ symptoms, prevention of adverse drug events and improvement of patients’ quality of life. At the Burnsville clinic, the pharmacist will support population health initiatives through the improvement of Medicare Star measures in collaboration with a pharmacy technician. The areas of focus will be on improving medication use related to polyanticholinergic burden, statin use in targeted populations and improvement of key adherence measures. Once credentialed, the pharmacist will also provide direct patient care through the provision of comprehensive medication management.

Requirements

  • Graduated from an accredited college of pharmacy on or after May 1996, or completed a structured and comprehensive education program approved by the Accreditation Council of Pharmacy Education (ACPE) and the Board of Pharmacy with didactic and clinical elements upon hire.
  • Two years of recent clinical pharmacy experience in ambulatory care as part of a multidisciplinary team within the last five years, or successful completion of one year specialized post-doctoral residency.
  • Current Minnesota Pharmacist license and maintain that license in good standing.
  • Deep knowledge of evidence based clinical guidelines, pharmacology, pharmacodynamics, and pharmacokinetics, pathophysiology and therapeutic disease states commonly found in ambulatory patients.

Nice To Haves

  • Board Certified Ambulatory Care Pharmacist (BCACP) or Board-Certified Pharmacotherapy Specialist (BCPS) preferred.
