About The Position

TriHealth is actively recruiting for a Pharmacist to join the PACE of Cincinnati PACE Center located in Norwood Ohio. Program of All-Inclusive Care for the Elderly (PACE) is a Medicare and Medicaid program that helps people meet their health care needs in the community instead of going to a nursing home or other care facility. This unique opportunity is for a full-time position, M-F 11:30 AM- 8:00 PM to service in a position to support PACE of Cincinnati and Hospice of Cincinnati as a clinical and dispensing pharmacist. The PACE Clinical Pharmacist is responsible for completing comprehensive initial medication management assessments for new PACE participants and medication management reassessments on existing PACE participants. For Hospice of Cincinnati, as the dispensing pharmacist, drug distribution skills including order entry, clarification if needed, accuracy of checking, technician supervision and timely delivery. Effectively manage day to day operations, handle initial conflict management, provide operational feedback and opportunities, implement corporate and departmental directives, and manage expenses within budget. Oversees and assists in the development of service-oriented staff through managing effective workflow and actively participating in the training of new employees.

Requirements

  • Bachelor's Degree Bachelor's of Science or Pharm D with current license in State. (Required)
  • One year of experience with a frail, elderly, or long-term care population.
  • Experience caring for frail older adults' physical, mental, emotional, and social needs.
  • Demonstrates the ability to work in an interdisciplinary team setting effectively. (Required)
  • Basic Life Support (BLS) Required

Responsibilities

  • Completing comprehensive initial medication management assessments for new PACE participants and medication management reassessments on existing PACE participants.
  • Drug distribution skills including order entry, clarification if needed, accuracy of checking, technician supervision and timely delivery.
  • Effectively manage day to day operations, handle initial conflict management, provide operational feedback and opportunities, implement corporate and departmental directives, and manage expenses within budget.
  • Oversees and assists in the development of service-oriented staff through managing effective workflow and actively participating in the training of new employees.
  • Completing a comprehensive initial medication management assessment for new PACE participants and medication management reassessments on existing PACE participants as part of the PACE IDT care planning process.
  • Develop participant care plans for medication management considering the medical, physical, social, and emotional needs of PACE participants.
  • Acts as a liaison between the contracted pharmacy provider and PACE provider staff to ensure the safe and effective delivery and distribution of medications to PACE participants.
  • Assures safe and effective medication management protocols by providing industry best-practice drug therapy monitoring regimens and ongoing medication management education to participants and caregivers.
  • Responsible for the medication management of PACE participants across the care continuum, including community settings, hospitals, and skilled nursing facilities.
  • Provides medication management support and guidance during care transitions from the hospital and skilled nursing facilities back to community settings.
  • Completes and maintains medication management documentation in the PACE EHR in accordance with the federal PACE regulation and organizational policy.
  • Provides medication management education to participants, caregivers, and PACE staff.
  • Attends IDT and morning meetings and reports on participant’s medication management status.
  • Participates in the Fraud, Waste, and Abuse Committee.
  • Supports the PACE QI program and participants in PACE QI activities as assigned.
  • Assumes personal responsibility for professional development, particularly in the area of ongoing clinical education.
  • Demonstrates a working knowledge of computer operations, all third party plans, ensuring pharmacy support staff is knowledgeable in the same areas, trouble shooting, and call resolution.
  • Demonstrates initiative in accepting additional projects.
  • Seeks areas of improvement.
