About The Position

This is a full-time (.9 FTE) Pharmacist position with varying shifts. The role involves preparing and dispensing medications, designing and monitoring pharmacotherapy, responding to drug information requests, educating patients and caregivers, ensuring continuity of pharmaceutical care, managing medication errors and medical emergencies, and adhering to medication-use policies. The pharmacist will also provide in-service education, participate in the formulary process and policy development, contribute to organizational goals through committee work, and ensure compliance with all accreditation, legal, regulatory, and safety requirements. Additionally, the role includes participating in performance improvement programs, managing time effectively, maintaining a professional image, attending staff meetings, and utilizing pharmacy technologies like automated dispensing units (Pyxis).

Requirements

  • Knowledge of pharmacokinetics, drug utilization review, drug distribution systems.
  • Excellent verbal and written communication skills.
  • Possess a current Oregon Pharmacy license or eligible for reciprocity.

Responsibilities

  • Prepare and dispense medications using appropriate techniques and following the hospital’s policies and procedures.
  • Design, recommend, monitor, and evaluate patient-specific pharmacotherapy.
  • Provide concise, applicable, and timely responses to requests for drug information from healthcare providers and patients.
  • Provide medication-use education to patients and caregivers.
  • Utilize pharmacy support personnel effectively.
  • Ensure continuity of pharmaceutical care to and from the acute and ambulatory patient-care settings.
  • Participate in the established process for assessing, managing, and reporting medication errors.
  • Participate in the management of medical emergencies.
  • Assume responsibility for the organization’s ongoing adherence to its medication-use policies.
  • Provide in-service education to physicians, nurses, and other practitioners as needed.
  • Participate in the hospital’s formulary process.
  • Participate in the development or modification of policies for the use of medications in the hospital.
  • Contribute to the achievement of the organization’s goals through effectively participating in or leading committees and work groups.
  • Comply in all activities with accreditation, legal, regulatory, and safety requirements.
  • Participate in the departmental performance improvement program.
  • Manage time effectively to fulfill practice responsibilities.
  • Maintain a professional image.
  • Attends Pharmacy staff meetings.
  • Maximize work efficiency through the use of computers and other technologies, including automated dispensing units (Pyxis).
  • Performs other duties as assigned.
